What we often see happening
Predictable Lateness
The inevitable result of a system where capacity is ignored. In knowledge work, when new tasks are launched faster than they are completed, the queue grows exponentially. Lateness becomes "predictable" because the mathematical reality of the backlog makes it impossible to meet original deadlines, regardless of how hard the team works.
High Context Switching
Occuring when people are forced to jump between too many disparate tasks or "Priority Pinball" scenarios. Because knowledge work requires deep mental focus, every "switch" incurs a significant cognitive tax. This fragmentation of attention creates the illusion of being busy while actually stalling the progress of any single piece of work.
Slow Pace of Output
The visible drop in the volume of completed work reaching the customer. This is often caused by "Flow Fog," where the actual path to completion is obscured by too much work-in-progress. Even though everyone is active, the system's "throughput"—the rate at which value is actually delivered—slows to a crawl.
High Waiting Time
It represents the vast amount of time a project spends sitting idle, waiting for an approval, a decision, or a specialist's input.
“Zombie” Projects
Initiatives that are neither officially dead nor truly moving forward. They continue to consume mental energy, "Pipeline Pressure," and status-report time without ever delivering value. These projects often survive because the organisation lacks a clear mechanism for "killing" low-priority work, leading to a cluttered and stagnant portfolio.
